  • the rubber wheelchair ramp needs patching, particularly where the concrete underneath is exposed. 
  • the play equipment is fine​​ except for a few areas of exposed metal on the decks and stairs

site evaluation
  • class 1 hazard (fixed) head and neck entrapment - the space between the deck and the top of the crawl thru activity panel was large enough for a child torso to pass through but too small for a child's head.  this created a possibility for strangulation.  (fixed during equipment maintenance) (pic 1)
  • class 1 hazard (fixed) bolt threads -  half a dozen bolts below the playground deck exceeded the 2 exposed thread limit.  the result is a entanglement hazard.  (fixed during equipment maintenance) (pic 2)
  • astm signage - updated
  • not ADA compliant - with 8 elevated elements, 3 ground-level elements (3 types) are required. the playground needs 1 additional ground-level activity. the manufacturer has been contacted for replacement parts pricing.
  • chain coating cracking- on the 'cliff climbers' both plastic coated chains are splitting,  the manufacturer has been contacted for replacement parts pricing - (pic 3)
  • rubber ADA ramp holes - there are 2 small holes and one larger hole that has worn through to the concrete.  this creates the possibility for head injury from falls and also twisted ankles. (pic 4-5)
  • benches broken - there are 3 tree ring benches.  2 of them are falling apart.  we hammered down the loose nails during the maintenance service. repair quotes provided (pic 6-7)
notes
  • periscope bolts - most of the bolts on the periscope were loose and required tightening.  since the activity is able to move during play, this may result in bolts coming loose in the future.  nothing needs to be done about this, we just want to make note of it to be sure to tighten everything during our next maintenance cycle. (pic 8)
  • the roots on one of the trees might pose a tripping hazard.  however, they also seem like they might offer great play value for sand, water and nature. the kids obviously are digging and interacting with them. they are outside of the use zone of the structure and don't seem to be in the path of travel. sandboxes with treasures to expose are quite popular and this may serve that same purpose. so long as they have not been observed to create a pattern of injuries, they may be a great asset for the play space. (pic 9)
  • ​exposed metal is showing through on some of the stairs and decks.  decks can be recoated or replaced to address this issue (pic 10)
  • some of the entrance barrier panels are a little warped.  nothing has snapped and no entanglement hazards have been created as a result.  just something to keep an eye on (pic 11)
